Creative Health: UK State of the Sector Survey
This survey is for everyone who is involved in doing, commissioning or supporting creative health work in the UK.

This is for volunteers, freelancers, self-employed people, and people employed in organisations in the UK. (You can find the survey in Cymraeg/Welsh here.)
We’re using ‘creative health’ to mean all work with creativity, arts and/or culture that supports health and/or wellbeing. (If you’re not sure whether your work fits you can see lots of other examples here.)
There are two parts to this survey:
- Part 1 is about your work with creative health
- Part 2 is an Equality, Diversity and Inclusion form.
